Our team of specialized nurses offers early intervention, education, therapeutic care, and system navigation, all tailored to meet the unique needs of each client. Services can be delivered in the home, in community settings, or through virtual care, depending on your preferences and needs.
Who Can Benefit From This Service?
This program is ideal for individuals who:
- Are experiencing mild to moderate mental health concerns (e.g., anxiety, depression, mood disorders)
- Need support managing medications or treatment plans
- Are transitioning from a recent hospital admission or crisis
- Require short-term stabilization support while accessing long-term care
- Have co-existing physical and mental health conditions that require coordination of care
Whether you or your loved one needs ongoing support or crisis intervention, our mental health services provide flexible and sensitive care that promotes recovery, independence, and overall well-being.
Our Services Include:
- Comprehensive mental health assessments and ongoing monitoring of symptoms and wellbeing
- Collaborative development of personalized wellness and recovery plans
- One-on-one clinical nursing support in the home or community

How to Access Our Services:
Referrals are welcome from individuals, families, physicians, therapists, or other healthcare providers. Once referred, a mental health and addictions nurse will be assigned to develop a tailored care plan in collaboration with the client and, if appropriate, their support network.
