Family-Managed Home Care is a unique program that empowers eligible families to take charge of their loved one's care by hiring caregivers directly—often from a trusted circle of family, friends, or personal networks. As a licensed nursing agency, AC Nursing works closely with families to provide professional guidance, staffing support, and care planning.

FMHC is a program available in Ontario that allows individuals to receive government-funded care services in their home—while giving their family or substitute decision-maker control over who provides that care.

Under this model, the family manages their own support team and is responsible for hiring, training, and scheduling caregivers. The program provides flexibility and choice, particularly for clients with complex care needs or cultural/language preferences.

We act as a partner to both the client and the family, ensuring care remains consistent, respectful, and in line with the FMHC program's expectations.
